This page is intended for customers who have already placed a repair order and need to complete this page to include with their orthotics. If you have not already purchased one of our services, please return to the Local "Place Order" page and complete your repair order.
Duplicate Shells
Current in-lab turnaround time: 3-5 business days
Once your repair order has been placed and your PayPal confirmation email has been received, fill out and print the customer information form.
Place your orthotics along with the completed customer information form in a Ziploc bag or other secure package.
Bring your orthotics to one of the approved drop off locations listed on this page, you will be contacted once your orthotics are ready for pick up.
